COVID-19 Health and Safety Protocols and Liability Release Waiver Health and Safety Protocols

Please know that the staff at Carp Village Tutoring is taking every precaution to avoid the spread of COVID-19. We remain committed to the health and safety of our clients, their families and our staff. Below are the protocols we have put in place to help eliminate the spread of COVID-19. These protocols have been carefully designed based on the recommendations and regulations provided by the Ontario Government and Ontario Public Health officials. If you have any question or concerns, we encourage you to reach out to the staff and bring it to our attention so that we can ensure the health and safety of everyone.

Entering and Exciting the Centre

As there are other businesses in the building we kindly ask that students be dropped off and picked up outside (unless parent supervision is required) to minimize the number of people inside the building. As the student approaches the building we ask that they are wearing a mask and respect the 2 metre (2 arm lengths) social distancing measures.

If a parent/guardian has a meeting with a staff member then please follow the procedure above for students.

For each person coming into the building we ask that you kindly use the hand sanitizer provided before entering the Centre and again upon leaving.


Liability and Release Waiver

COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, Carp Village Tutoring (“the Centre”) has put in place preventative measures to reduce the spread of COVID-19; however, the Centre cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Centre could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Centre and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Centre may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Centre employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the Centre or participation in Cenere programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Centre, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Centre, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Centre program.
